Registered Nurse Assessment Coordinator (RNAC) Responsibilities:
Development, coordination and completion of resident assessments
Preparation of the MDS report generation and analysis
Data transmission and development of care plans
Assuring compliance with Federal and State regulations governing long term care
Working closely with the inter-disciplinary team
Registered Nurse Assessment Coordinator (RNAC) Qualifications:
Active RN license
Minimum of 2 years of RNAC experience in long-term care
Knowledge of MDS Assessment process, Medicare, and long-term care federal and state regulations
Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resource utilization and/or case management is highly desirable
RAC-CT certification, preferred
