Kaiser Permanente is seeking a Visiting Nurse ( RN ) - Home Health to join our fantastic Home Health team in Seattle, WA!

This this is a 1.0 FTE position, generally working Monday - Friday and generally an 8:00am - 5:00pm work schedule.

Rotating/alternating weekend schedule is involved and shared among the team.

The Visiting Home Health Registered Nurse will:

* Be part of an inter-disciplinary team, which may include RNs, Medical Social Workers, Spiritual Hospice Counselors, Physical or Occupational Therapists, Speech Pathologists, MDs, Home Health Aids and a caring Clinical Team Manager!

* Work 1:1 with patients to provides comprehensive skilled nursing care and services to patients in the home setting

* Work on an interdisciplinary team to provide care in the home health, palliative care and hospice programs

* Be accountable for the assessment, development of plan of care, education and consultation related to pain and symptom management as well as adjustment to changes in health status

* Plan for the return of patient care to primary healthcare providers as indicated

Qualifications

Education

Minimum: Bachelor's Degree

Preferred: Bachelor's of Science in Nursing or related healthcare bachelor's degree

Knowledge/Skills

Minimum: Intermediate ability to work independently; ability to transfer/position patients as needs require; basic laptop skills including MS Office Suite; customer service skills; demonstrated experience providing care planning within a community setting (continuum of care)

Preferred: Basic or limited McKesson for homecare system experience.

Experience

Minimum: 2 years of experience as a Registered Nurse in medical surgical or home health

Preferred: 2 years home health or hospice

License/Certifications

Minimum: Current Washington State RN License and Washington State driver's license with proof of insurance by time of hire
