The Home Health RN provides quality care and coordinates home care to clients served by a Primary Care Team under the direction of the RN supervisor. The Home Health RN supervises and delegates to the Home Health LPN and Home Health Aide/CNA appropriately and coordinates with other team members, the physician, and clients. 




*** This is a full time position with 8-5 hours and weekends are a must. Must be available to do call and evening visits as necessary***


Primary Responsibilities:





  • Coordinates with the RN supervisor to assure the client driven plan of care strives to meet positive outcomes through on going evaluation using evidences based practice


  • Responsible for making certain the home care provided is compliant with State and Federal regulations (documentation and delivered) through participation in chart review and following established process


  • Responsible for participation in orientation, on-going education programs and successful completion of competency requirements


  • Responsible for providing services that are congruent with the policies of FHS within the scope of professional nursing practice as defined in the Nurse Practice Act


  • Strives to provide services to meet both internal and external customer expectations


  • Assumes individual responsibility for professional competence and professional nursing practice


  • Assures that the services provided are efficient and of value, meeting the expectations of quality healthcare delivery within operations goals of the Division, FHS and United


  • Ensures home care is proved in accordance with agency and United Corporate Compliance Program


  • Majority of job duties are carried out in patient's homes and driving throughout the community


  • On call, weekends and rotating holidays as needed







Requirements

Required Qualifications: 




  • Current unrestricted RN license in the state of Nevada
  • 1-2 years of acute care experience required. (i.e. Medical Surgical, Critical Care, ICU) or several years of home health experience will be considered
  • Must  have reliable transportation, possess a current Nevada driver's license and maintain personal insurance coverage
  • Must be willing to travel within the Western region of Las Vegas providing direct patient care in the home setting
  • CPR/BLS is required
  • Must be able to work weekends and participate in on call rotation and work occasional evening visits when needed

  • Must be able to lift up to 50lbs and be able to perform these functions on a regular basis, such as sitting, standing, walking, bending, crouching, twisting and reaching



  • Must be CPR certified

Preferred Qualifications:



  • Home Health experience highly preferred

  • Knowledge and competent with IV drug administration, blood draws, wound care and use of wound vac. IV certification



  • Bilingual Spanish speaking is a plus!


Knowledge: Knowledge of professional standards of care, and laws and regulations applicable to home health services. Technically proficient in field of practice.

Skills: Demonstrates key technical skills utilized in the day-to-day management of home health patients. Must be able to work independently, and posses basic computer skills. Ability to develop and implement plan of care based on assessment skills and demonstrates independent clinical and problem solving skills.
