We are seeking knowledgeable, dedicated and compassionate RNs to provide direct care to hospice patients in their homes and/or nursing facilities. As a Hospice RN, you will work with other healthcare professionals to determine the best overall care for your terminal patients (prognosis of 6 months or less). Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.

 

Our Optum Palliative and Hospice Care offices in Reston and Haymarket are hiring!!

 

We have a variety of Hospice RN opportunities available including full time, part time and PRN positions!! Positions include RN Case Managers, PRN RNs and On Call/Triage RNs!

 

Please note: Our Hospice RN positions are field based and require travel throughout our service area to provide quality care to our hospice patients and their families. Participating in an overnight on call rotation is required.

 

Responsibilities for the Hospice RN Case Manager include:


  • Conducting regular visits at home or nursing facilities for a caseload of hospice patients

  • Coordinating, assessing, and implementing patients' care

  • Providing accurate and timely documentation of patient services to reflect plans of care

  • Overseeing and participating in day to day clinical care of terminal patients and their families

  • Attending weekly interdisciplinary meetings

  • Interacting with other group members such as CNA's, LPN's, social workers and dieticians

  • Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials

  • Participating in a required on call (overnight) schedule that rotates throughout the team

Requirements

Required Qualifications:



  • RN licensure (current, unrestricted) in the state location of position

  • 3 years of clinical experience

  • Reliable transportation including valid driver's license and auto insurance

 


Preferred Qualifications:



  • Excellent communication skills

  • Experience in long-term

  • Home care and/or hospice care experience

  • Computer proficiency with MS Office products including Word and Outlook

  • Proficiency with electronic medical records (EMR) software
