Illinois MENTOR is seeking an independent, self-motivated Program Services Coordinator to coordinate the development and implementation of services and treatments for adults with intellectual and developmental disabilities receiving supports. This position will require routine travel within Chicago and the surrounding area.
Program Services Coordinators oversee all services provided to individuals, monitors quality of care delivered, and monitors corporate and regulatory program compliance.
Responsiblities include but are not limited to:
- Maintain current records for all independent contractors on caseload including training, monthly and annual mentor contract compliance forms, respite and support plans and tracking of respite support workers
- Assist in independent contractor inservices, planning and development.
- Maintain current and complete client records and reports in accordance with corporate and regulatory standards
- Assess referred clients and coordinates the development of a service plan in conjunction with a treatment team and funding source in accordance with corporate, contrctual and regulatory standards. Coordinates and evaluates plan implementation.
- Coordiantes the development, review and revision of programs and behavior intervention plans. Provides, monitors and assures that comprehensive support services and oversight are provided on an ongoing basis
Requirements:
- Bachelor’s degree in human services or related field and one year of related work experience working with adults with intellectual disabilities preferred
- QIDP/QMRP
- Strong written and verbal communication skills
- Strong computer skills
- Valid driver’s license in good standing
- Car registration and vehicle insurance if providing transportation for individuals receiving services
- Successful clearance of local and/or state background checks
