In this position you will be identifying members with housing needs and interfacing with health homes to identify housing resources to support members and coordinate care. You will be utilizing the telephone and outlook to connect with these members and assist with their housing needs. Members will be identified from multiple areas and will be passed along to your desk. You will also assist with discharging coordination when needed. Other duties as assigned.
Requirements
- High School Diploma/GED.
- Travel - 75% to local offices.
- 1+ year of experience working with a computer and telephone as the primary instruments to perform the job duties.
- 1+ year of experience working with Microsoft Office.
- 6+ months of experience working with individuals with a Mental Illness or substance use disorder.
- 6+ months experience working independently, with little supervision.
- Experience working with the New York department of homeless service.
- Knowledge of or the ability to research and connect with community resources.
