About this job
Registered nurse coordinating care for patients receiving treatment for chronic diseases/disorders in their home. Assures the provision of nursing care by coordinating care with competent and trained home health agencies. Finds and coordinates quality home care services, provides disease and therapy education of home care professionals, and provides patient education. Monitors participants progress on therapy. Acts as the liaison between Caremark staff and the home health agency staff. Interacts with treatment centers, physician, payors, and other healthcare professionals to facilitate the coordination of a referral or subcontracted nursing home care visits. Participates in entry of nursing agencies and home care visits into the nursing network database.
Required Qualifications Registered nurse (RN), with current valid nursing license in the state of employment.
Minimum of five (5) years recent clinical experience with two or more (2+) years of home care and/or infusion experience preferred.
Preferred Qualifications 1. Knowledge of and experience with infusion therapy supplies and pumps.
2. Demonstrate ability and initiative to work independently in managing the delivery of participant care and as part of a coordinated team.
3. Excellent interpersonal, verbal, and written communication, time management, organizational and customer service skills.
4. Demonstrate accountability and accuracy in assigned tasks along with effective problem solving ability.
5. Ability to become licensed in all 50 states, as necessary.
Education Graduate of an accredited registered nursing program, BSN preferred.
