About this job

*Assess and monitor the overall needs of patient/significant others. Acts as patient/family advocate. Provides observation, evaluation, and hands on skilled nursing care under a physician's plan of treatment to meet the physical and psychosocial needs of the patients and reports changes to supervisor as appropriate. Performs nursing procedures in specialized areas as it pertains to treatment of medically complex patients.

*Contributes to the coordination of services in conjunction with other members of the health care team as appropriate; may attend care conferences as requested.

*Assesses patients and initiates needed nursing services; makes appropriate referrals to other disciplines.

*Observes patient for changes in condition; reports changes appropriately to the physician or supervisor; obtains orders for changes in the medical plan of treatment to respond to patient's conditions, and documents all observations and interventions in an accurate and timely manner.

*Initiates and applies appropriate preventative therapeutic and rehabilitative procedures and techniques. Maintains communication regarding the patients care with physician and supervisor as appropriate. Coordinates with vendors of specialized equipment/supplies necessary for care of complex medical conditions.

*When assigned, supervises ancillary personnel. May provide transportation of client to therapy, medical, or social appointments in the staff/client's vehicle.

*Assists and instructs clients' family members, field staff, or other client representatives in nursing care and other concepts and activities necessary to promote the clients health, safety, and independence.

Basic Qualifications:

Education - Graduate from an approved school of Nursing

Experience - 6 months of experience in as an RN prior to agency assignment

Licenses - Registered Nurse (RN)

Drivers License -- MI (MI DR LIC)

Cardiac Pulmonary Resuscitation (CPR)

Preferred Qualifications:

Experience - 1 year of experience in a rehabilitation or health care setting
