The Therapist is responsible for providing clinical services to patients and their families, as a member of a multidisciplinary team and in accordance with Dominion Hospital's social work policy. Clinical interventions are focused on assessment, crisis stabilization, psycho-education, and disposition planning. Services provided must be for the purpose of diagnostic study or reasonably be expected to improve the patient's condition The treatment at a is minimum designed to reduce or control the patient's psychiatric symptoms so as to prevent relapse or hospitalization, and improve or maintain the patient's level functioning.

Specific Job Function:

* Provide patient training and education to the extent the training and educational activities are closely and clearly related to the care and treatment of his/her diagnosed psychiatric condition

* Completion of initial assessment upon patient admission.

* Thoroughly complete individualized treatment plans within three (3) of the admission to the program.

* Completion of psychosocial assessment no later than third scheduled treatment day.

* Thoroughly completes discharge plans.

* Review safety plan with the patient at the beginning and end of every treatment day and update the plan as needed and document appropriately.

* Complete treatment plan interventions with signatures.

* Provide individual/family sessions appropriate to client needs.

* Provide group therapy appropriate to population.

* Attend and participate in treatment planning meetings.

* Collaborate with attending physician and nursing staff regarding safety, treatment and discharge planning.

* Collaborate with referral source and continuing care providers.

* Display professionalism in dealing with customers, staff and clinicians.

* Complete assignments in a timely manner.

* Complete documentation in a timely manner.

* Follows all hospital policies and procedures.

Education Master's Degree from an accredited social work program or Master's Degree in a related field, which provided appropriate background and experience. Education should be clinically focused with study in human life span development and psychopathology.

Certifications CPR-BLS for healthcare providers required. CPI(crisis prevention interventions) required as a part of the general hospital orientation.

License candidates to possess LCSW or, LPC or other appropriate licensure, background and experience.
