As a Home Visit Nurse Practitioner with Optum, you'll provide primary care home visits for the patients in collaboration with the physician of record and patient care team. After conducting a comprehensive patient assessment, you will communicate and manage the patients' care across all care providers, family, and caregivers to ensure that timely, patient-centered, appropriate evidence-based medicine is provided. By building strong relationships, you'll have a meaningful impact on the patient's health.

Primary Responsibilities:


  • Identify and develop innovative actions to meet the needs of OPTUM patients from both the health care and psychosocial / socioeconomic dimensions of care
  • Take action for provision of services to meet the patient needs
  • Conduct comprehensive physical assessments
  • May be required to develop risk management plans, provide maintenance care and acute interventions within scope
  • May conduct procedures such as IV access and wound care (not all inclusive)
  • Provide patient education and complete CMS compliant documentation in an EMR
  • Ensure all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS, OPTUM and client requirements

Requirements

Required Qualifications:


  • Active, un-encumbered license to practice nursing in the state of assignment or ability to obtain
  • Nurse Practitioner Certification from either the ANCC or AANP or ability to obtain
  • Minimum of 1 year experience preferred
  • Must be able to effectively communicate with elderly and chronically ill patients and families
  • Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial
  • Comfortable working independently in a home-visit setting
  • Basic knowledge of computers and cell phones
  • Familiarity with ICD-9 and CPT-4 coding
  • Basic Cardiac Life Support (BCLS) Certification
  • Excellent organizational skills with the ability to multi-task
  • DEA License or the ability to obtain
  • Required participation in the Patient Care Team on-call schedule

Preferred Qualifications:


  • Geriatric or adult medicine specialty
  • Bi-lingual or multi-lingual
