The Patient Care Coordinator RN is responsible for overseeing and facilitating patient-centered care and services through the full continuum of care across inpatient to outpatient settings for identified orthopedic patients. The position's emphasis will be on care coordination, system analysis and positive partnerships with patients, families, interdisciplinary teams, physicians and community resources, in order to provide quality service outcomes. The Ortho Navigator will support the multidisciplinary efforts of process improvement to achieve Orthopedic Specialty Accreditation and recognition as a Center of Excellence. The incumbent will serve as a patient advocate for the patient and family by identifying and valuing patient choices, spiritual needs, cultural, language and socioeconomic barriers.

Education:

Required

Bachelors degree.

Special Training: (including license):

Required

Current and valid California RN license.

Working knowledge of the RN scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, Patient Bill of Rights, AB 1203 and other federal or state regulatory requirements as specific to Utilization Review and Discharge Planning.

Fluent in English verbal and written skills.

Working knowledge of Microsoft Office and work related software.

Preferred

Certification appropriate for the position (CCM, ABQAURP or ACM).

Bilingual skills.

Experience: (minimum, plus desired):

Required

Two years nursing experience in an acute care hospital.

Preferred

Two years case management in an acute care hospital.
