RN Case Manager Job

RN Case Manager needed for a permanent opportunity with Yoh's client located in San Rafael, CA

The Big Picture - Top Skills You Should Possess:

- Case Management Experience
- Hospice Experience strongly preferred

What You'll Be Doing:

- Manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention
- Demonstrate experience developing and maintaining rapport with patients and families
- Perform other duties as assigned

What You Need to Bring to the Table:

- Current California RN license
- Associate's degree in Nursing required
- At least two (2) years of experience in a hospice or home health clinical setting
- Excellent written and verbal communication skills
- Ability to prioritize and efficiently manage multiple tasks
- Ability to develop and maintain rapport with the client, families and staff
- Excellent Computer and Microsoft Office Application skills
