This position is based Labelle and reports to the Ryan White Part C Program Manager Operations and Management Consultant II -- SES.

Job Specific duties/responsibilities include:
I. Manages and treats adult patients in accordance with an established collaborative practice agreement using established DOH protocols, policies and procedures.
Evaluates and manages patients in the primary care setting in accordance with current ARNP professional practice standards, evidence based standards, and medical treatment guidelines and in consultation with supervising physician when indicated.
Documents all observations and actions accurately and in accordance with program guidelines, including prescribed timelines.
Collects data pursuant to and evaluates patient complaints, medical histories, and physical findings.
Orders diagnostic laboratory tests for evaluation and management of patients. Interprets and evaluates laboratory results.
Provides preventive health care services including annual or periodic comprehensive physical examination utilizing various techniques such as observation, palpation, percussion, and auscultation.
Evaluates and initiates or modifies the treatment plan of common medical conditions in accordance with scope of practice
Prescribes medication according to scope of practice and accurately and legibly documents medication in the medical record
Provides referrals for physician or specialty care when indicated.
Provides information/education to patients and/or their families upon a diagnosis, including a plan of care
II. Initiates and manages contraceptive methods for Family Planning patients after evaluation, history,
laboratory tests, physical assessment and examination.
Provides verbal and written instructions regarding method.
Obtains informed consent.
Performs reassessment, screening, and +counseling.
Provides pre-conceptual and inter-conceptual health services.
Provides non-prescriptive methods.
Provides pregnancy testing and counseling.
Manages and treats sexually transmitted diseases.
Provides follow-up for abnormal pap smears.
Provides sterilization counseling
III Responsible for providing services to HIV patients utilizing resources provided by the Ryan White Part B and C grants.
Serves in a leadership role related to quality assurance and quality improvement.
Responsible for organizing the collection of data, ensuring the integrity of the data, and using the data in a structured manner to establish benchmarks
Contributes to increasing access to HIV medical services through collaboration with clinical and clerical staff
Refers to, supports, and collaborates with medical case management and Ryan White case manager to ensure HIV patients obtain access to mental health, nutrition, oral health, and substance abuse services
Evaluate and manage HIV patients in accordance with medical treatment guidelines and in consultation with Physician of the HIV Program
Lead quality improvement initiatives related to the Ryan White Part C grant
Conduct chart reviews
Provide in-service education
Lead staff development initiatives using established QI strategies
lV. Participates in Quality improvement activities, including chart reviews, in-service programs, and staff
meetings.
