Rivendell Behavioral Health Services is a 79-bed acute care facility located in Benton, AR , has been providing quality health care to the residents of Arkansas since 1986. Rivendell offers access to a full continuum of emotional and behavioral health services for adults, children and adolescents. Our multi-disciplinary treatment approach has created an environment for healing. Rivendell also provides outpatient services at 12 locations throughout Arkansas. Rivendell currently has a Registered Nurse position available. The candidate selected will be responsible for the following:
RN Weekend
The RN has responsibility for creating, maintaining and managing a therapeutic milieu within the treatment program to ensure the safety of patients and staff. This includes being in charge of the MHA's, by making assignments and monitoring the care which the MHA's give. Performs ongoing assessments of the behavioral and medical needs of the patients each shift. Charts those observations of the patient in the nurse's note each shift, in a legible and concise manner. Plans, implements and evaluates age specific and developmentally appropriate individualized nursing care on the interdisciplinary treatment plans. Updates treatment plans as necessary. Demonstrates knowledge and competency in age-specific criteria of the unit to which assignment is made:
Adult Acute Care Unit (short term stays for general adult population above age 18)
Assists with activities of daily living as indicated for the specific unit including safety issues related to immobility, safety, restraints, and decreased sensorium. Is able to assess the patient's level of orientation and identify cues for assessment based on functioning level. Understands the specific unit programming and functioning level interventions to meet the needs of the more chronic patient.
Acute Care Unit (short term stays for ages 5 through 17)
Understands the difference between age appropriate behaviors versus pathological behaviors in the patient population. Understands the uniqueness of the program tract and the focus on development status. Adolescne Long Term Care: Assists with activities of daily living as developmentally indicated for the specific unit including safety issues related to immobility, safety, emergency interventions, and decreased sensorium. Is able to assess the patient's level of orientation and identify cues for assessment based on developmental theory. Understands the specific unit programming and developmentally sound interventions to meet the needs of the more chronic patient. C arries out physician orders regarding diet, medication and treatment. Report to the charge nurse patient's signs, symptoms, and behaviors and any subsequent treatment and/or interventions and the patient's response to the treatments and documents same in the medical record according to hospital policy. Alerts physician to clinically pertinent findings in a timely manner. Under the direction of the Charge Nurse authorizes emergency implementation of seclusion and physical restraint in the physician's absence according to hospital policy. Participates in, supervises and documents, special treatment procedures, progressive interventions and de-escalation of patients according to hospital policy. Reviews all Emergency Intervention documentation, and initials all entries made by the MHA/LPN or other staff. Completes the QA information on each STP sheet. Assists in coordinating treatment as a team member through participation in treatment planning meetings. Supervises and/or provides direct supervision to those patients on 1:1 precautions and assures the obervations levels are followed. Understands and teaches others how to properly transfer observation check sheets to on coming staff. Educates patient/family toward an understanding of health/illness and the plans for post discharge care. Responsible for running or supervising all milieu groups and health teaching groups. Attends mandatory hospital and department inservices and monthly department meetings. Participates in the Performance Improvement process and other research projects. Performs basic first aid, manages emergency situations as necessary. Administers PRNs to assigned patients and completes appropriate documentation on the MAR. In addition: if working the 11/7 shift, reviews charts nightly ensuring that all orders are on the MAR, completes night chart audits on all new admissions and programs to program transfers and participates in monthly QA programs. Performs other duties as assigned.
Must be a Registered Nurse licensed to practice nursing in the State of Arkansas. CPR certification and Handle with Care must be current - training will be provided on site.
Must have the ability to supervise LPNs/LPTNs/MHAs and ancillary staff in selected aspects of direct clinical care is required.

Ability to teach select aspects of nursing care to others. Ability to exercise sound judgment in evaluating situations and /or making nursing decisions. Ability to work harmoniously and effectively with patients, families, visitors and staff.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
