RN Case Manager for LTACH - Full-time / Part-time

Norman Specialty Hospital provides a range of services, including post-acute medical and rehabilitation care.

The RN Case Manager is responsible for accurately assessing patients for admission, including: Clinical needs, appropriateness for admission to LTACH setting, and assuring adequacy of financial support prior to admission to Hospital. Facilitates interdisciplinary plans and assures progress reports are completed and provided to payor on a monthly or as needed basis; Also serves as liaison between patient, physician, care team members, payor and the discharge planner by coordinating monitoring, and communicating patient's progress and cost evaluation while in the LTACH setting and assisting with coordination to next level: Assists Administrator with census management.

Educational Requirements:Education: C.I.R.S. or C.M.C.M. preferred. Current license as an Rn in state and CPR certification required. ACLS preferred.

Position Requirements:

Minimum three (3) years clinical experience and two (2) years experience in medical case management.
