The Client Care Coordinator is expected to perform a variety of duties that relate to client care including Quality Assurance Visits, Client/CAREGiver Introductions, and Care Plan Creation and Updating. They are responsible for continuously evaluating the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention, as well as seeking opportunities to increase service hours. Qualified candidates will have strong people skills, and strong administrative skills. The individual must be organized, self-motivated and empathetic. Candidate must work well with other team members in coordinating the various aspects of client care. Computer skills and the ability to create and maintain client and responsible party records which document all quality assurance meetings and client care plan updates is a must. Accountabilities include planning and executing a schedule that ensures each client is seen per companies goals and guidelines. Candidate must be able to recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients and keep these updated to account for any changes in client's circumstances. Open and effective communication with clients, family members, colleagues and CAREGivers is a must. Position is full time salaried that will occasionally require client visits on nights and weekends. Salary for this position is $500 per week. Benefits include 7 paid holidays + an additional 7 PTO days per calendar year. All employees of Home Instead Senior Care must pass an AHCA level 2 background screening and drug testing.
