JOB DESCRIPTION: A new clinician who continues to demonstrate growth in professional role by consistently using the Synergy Model and evidence-based research to provide care to assigned patients and their families. Demonstrates understanding of role of other disciplines and readily collaborates with them to meet assessed needs of patients/families.

Communicates effectively and respectfully with the patient, the patients family, and other members of the healthcare team. Delivers safe, consistent, and appropriate care to a patient population with guidance utilizing the nursing process. Demonstrates basic assessment skills and implements appropriate interventions. Demonstrates development in recognizing the impact of nursing care on patient outcomes. Recognizes own performance limitations and seeks assistance when necessary. Documents the nursing process accurately and in the appropriate format (e.g. computer, clinical pathway, chart) with assistance when necessary. Plans and implements patient/family centered care based on assessment and patient /family needs with input from all members of the healthcare team. Evaluates patient's response to planned care. Demonstrates development in identifying patient and family learning needs and utilizes resources to meet those needs. Demonstrates basic understanding of AACN Synergy Model. Demonstrates development in time management skills. Complies with patient policies and procedures. Applies evidence-based practice and research cited in policies and procedures to the development/modification of patient-family centered plans of care.

Demonstrates development of delegation skills based on the synergy model of care. Begins to recognize the roles of the interdisciplinary team, patient and family care partners and has basic understanding of their role as a resource in planning patient care. Participates in cost containment activities. Identifies and resolves conflicts with assistance. Functions as Charge Nurse only when assigned due to extenuating circumstances.

Recognizes individual patient satisfaction issues and adjusts patient care delivery accordingly when appropriate. Begins to recognize the difference in how patients and families react to illness and treatment. Asks for assistance to resolve complex issues. Respects and maintains confidential information.

Demonstrates growth in learning basic knowledge and skills related to practice setting. Verbalizes understanding of application of the Synergy model for care in clinical practice.

MINIMUM QUALIFICATIONS: External candidates must be a graduate of an accredited school of nursing with a Bachelor's Degree in Nursing. All candidates must be currently licensed as a registered nurse in the state of Georgia and have completed 12 months of recent nursing experience within the past five years. BLS, ACLS, and Healthcare Provider certifications required. Annual fit testing. Annual health assessment completed in month of hire. Completes annual regulatory/safety/compliance tests on-line Annual Skills Day completion. Attends 75% of dept-based meetings (defined by dept leadership). Meets attendance requirements. Achieves required points and contact hours as defined in the PLAN (clinical lattice) point system to maintain role.

PHYSICAL REQUIREMENTS: (Medium-Heavy) 36-75 lbs, 0-33% of the work day (occasionally); 20-35 lbs, 34-66% of the workday; (frequently); 10-20 lbs, 67-100% of the workday (constantly); Lifting 75 lbs max; Carrying of objects up to 35 lbs; Occasional to frequent standing & walking; Occasional sitting; Close eye work (computers, typing, reading, writing); Physical demands may vary depending on assigned work area and work tasks.
