SHIFT: 8HOUR, Day EMPLOYMENT TYPE: Part Time HRS PER PAY PERIOD: 40 JOB ID: 1500032637
The Linkage and Outreach Worker reports to CARE Program Managment. Working collaboratively with the Emergency Department, CARE Clinic and Mobile Clinic the incumbent ensures that patients are linked or re-linked to the CARE Clinic as necessary. The incumbent performs these duties as reflected in the mission of Dignity Health/SMMC and the CARE Program & Clinics.
Minimum requirement of one year working in a related position within a social services setting. Knowledge of public benefits programs a must. An understanding of HIV/AIDS helpful. Bachelors degree preferred. Must demonstrate strong organizational and communication skills. Bi-lingual (Spanish/English) desirable.
