POSITION ANNOUNCEMENT



POSITION: Day Habilitation Specialist



FT, Days, Mon-Fri  Positions available in Broome and Tioga Counties!



DUTIES AND RESPONSIBILITIES:The Day Habilitation Specialist will be responsible for delivering services in one or more core rooms operated by the Day Habilitation Program. These duties and responsibilities include, but are not limited to:



1. Actively cooperating with the Program Director and other program staff in order to achieve the goals and objectives of the Day Habilitation Program.



2. Responsible for the implementation of goal plans, data collection to monitor progress and monthly progress notes. Goal Plans will be based upon the person’s valued outcomes as identified in the person’s ISP.



3. Responsible for complying with all waiver Regulations, Part 624 and 633 in total, and reporting violations, deficiencies, and/or incidents to the Direct Supervisor as they are discovered or indicated.



4. Responsible for the development of individualized goal plans in accordance with the 3 IP, 5 governing principles and the person’s valued outcomes as identified in the ISP.



5. Complete all mandatory training in accordance with OPWDD and Agency Policy which includes, but is not limited to First Aid, CPR, SCIP, Medication Administration, 3 IP’s, 5 Governing Principals, Active Treatment, etc.



6. Perform other duties as outlined in the full job descriptions and as assigned by the Direct Supervisor.



SPECIAL REQUIREMENTS *



Candidate must be able to lift 50 pounds and not have any lifting restrictions which would prohibit him/her from assisting with toileting, transfers, physical intervention, lifting, etc.  Posses a valid driver’s license and good driving record acceptable to the agency’s insurance carrier. Maintain a CDL License.  Maintain certification in First Aide, CPR and SCIP.  Accountable for identifying, reporting and/or correcting all safety hazards including physical plant problems and/or problem work practices and investigating all work-related accidents.



QUALIFICATIONS



A Baccalaureate Degree in Human Services, Special Education or a related field and previous experience working with the Mentally Retarded/Developmentally Disabled  -Or- A combination of education and experience that meets the needs of the reviewing agencies.
