Program Support Coordinator-16000374 Job Program Services Coordination/Case Management Primary Location United States-NJ-NJSomerset Organization Hastings Description

New Jersey MENTOR is seeking a Program Support Coordinator to provide organizational assistance and clerical support to the Program throughout the participant programming and discharge.

Essential Job Functions

  • Attends Team Meetings and assists with agendas and reporting as required
  • Organize participant conferences including agendas, communication/scheduling, etc. with all significant others.
  • Coordinate inter agency referrals and service provisions including waiting lists, completion of forms, applications, etc.
  • Assist in internal communication to team members relative to case-specific issues.
  • Assist with identification and information gathering relative to designated discharge environment.
  • On a monthly basis, track billing, service logs, and services provided to participants as identified by the Regional Director.
  • Attends regularly scheduled supervision meetings with supervisor.
  • Other duties as assigned
Qualifications

Requirements:

  • High School Diploma or G.E.D. Equivalent
  • College degree preferred
  • Minimum of two years of related work experience.
  • Knowledge of New Jersey Division of Developmental Disabilities [(DDD) 10:44 A and 10:44 B] regulations is a plus

Core Competencies:

  • Dependability – Must be able to independently follow through with all aspects of programmatic assignments.
  • Team Orientation – Must effectively participate in an active multi-disciplinary therapeutic team environment and must actively be able to participate in program development.
  • Critical Decision Making – Must be decisive, confident and focused, and able to make good decisions related to program development, and assignments.

Other Skills:

  • Valid driver's license
  • CPR & First Aid Certified.
  • Complete other training as required
