PA Practical Nurse license required; New Grads welcomed!! Part-time 2:30-11pm for every other Sat/Sun/holiday; 16 hrs per week

Provides guidance to fellow team members, residents, residents' families, physicians, and other departments.  Provides leadership and supervision to Resident Care Assistants.  Provides clinical care to Residents in Personal Care. Responsible for oversight of Resident care in Personal Care.

 

Ensures consistency among Supportive Living Communities in all operations, policies and procedures.

 

Administers treatments and medication as specified in MAR, TAR, PRN sheets and follow properly when administering medications (orally or by subcutaneous or intramuscular injection, and notes time and amount on residents MAR as prescribed by physician).

 

Assembles and uses equipment including catheters, tracheotomy tubes and oxygen suppliers.

 

Assures resident care is completed by assignment of duties to RCAs.

 

Documents changes in resident care and condition in resident's chart.

 

Maintains and updates resident records with all pertinent information, including assisting social worker with Level of Living, Annual Assessment and Annual Support Plan.

 

Maintains calendar of resident appointments and assures that transportation of residents to appointments is arranged by Willow Valley Transportation, Willow Street Van Service or family, and document on calendar.

 

Assesses residents' condition and informs physician, family/responsible party of any changes and plan of action/treatment, as needed.

 

Receives orders from physicians and brackets order with time, date and initials.  Transfers order to current MAR, TAR PRN with date, time and initials.

 

Faxes pharmacy of changes in medication orders or need for refills, initial and date order changes.

Complies with Department of Public Welfare regulations by following policies manual on each floor.

 

Completes lab requests per physician orders with all pertinent information.

 

Reviews MAR, TAR, PRN sheets at the end of every month with new MAR, TAR, PRN.

 

Assures that residents are seen by a physician annually for medical evaluation or when change in resident's condition.

Assures that physicians complete necessary admission/annual paperwork, including medical evaluation, standing orders, admission orders (including TB testing, PT/OT/ST, etc.)

Builds team in a positive manner by supporting other team members, including team members in other departments.
