The RN participates as a member of the health care team in delivering quality health care to patients which supports KP's mission, vision & values. The RN provides a variety of nursing srvs both in person & by phone utilizing the nursing process & leadership skills to address acute & chronic needs of Health Plan members & other patients of the KP Health Care Program. He/she works under the general direction of the designated Sup'v.

ACCMP Overview:

The Anticoagulation Care Management Program (ACCMP) has been a group within Kaiser Permanente Northwest for over 30 years. Care is provided 7 days a week virtually to patients, with no patients seen in the clinic. This increases the convenience to patients, as they are able to go to any Kaiser lab for their blood work and we are able to reach out with the result. Our staff develops great relationships with patients as they understand their needs. Patients greatly appreciate the care that our staff provides and have a high level of satisfaction. In addition, providers are grateful for the ways in which ACC helps ensure high quality care. The ACCMP is an interdisciplinary team that works together to ensure proper patient care with a high level of teamwork to assure that patient needs are satisfied. This position will be at the Kaiser Permanente West Interstate Campus at 3325 N. Interstate Ave, Portland 97227 on the Lower Level. We continually evaluate our program workflow and our services to our members in order to ensure that we are working to the best of our ability and at the top of each person's license. ACC RNs work 8 hours a day, 5 days a week with other RNs, Pharmacists, LPNs, MAs, and Patient Access Specialists (PAS). A majority of shifts will be during daytime clinic hours with a standard schedule, with a rotating Saturday shift that is shared between 12 RNs. Exact shift times may vary slightly based on need, but there is a high level of consistency. The RN carries a load of patients and he/she manages their dosing of warfarin by INR results per protocol, but work is divided at the start of the day and the team helps each other at the end to ensure the work is completed. Nurses assess patient factors that may affect dosing and make decisions to optimize care. It requires a high degree of assessment over the phone, learning the protocols, great communication skills and the ability to work closely with others in a fast-paced team environment, with a large amount of responsibility for patient care. Experience with Epic, our electronic medical record, is highly desirable.

Basic Qualifications:

Experience

* Minimum two (2) years of nursing experience in acute care or ambulatory care/clinic setting within the last five (5) years.

Education

* Graduate of Accredited School of nursing.

* High School Diploma or General Education Development (GED) required.

License, Certification, Registration

* Current Oregon and Washington RN license required. License in state where assigned required upon hire/transfer, application for license in secondary state required within 2 weeks of hire/transfer and must be obtained within 6 months.

Additional Requirements:

* Demonstrates clinical nursing and leadership skills.

* Able to type 20 words per minute.

* Demonstrates effective interpersonal, communication and problem-solving skills.

* Willingness to learn computerized information systems.

* Demonstrates ability to work within teams and within a dynamic work environment.

Preferred Qualifications:

* Previous experience with population care/case management, triage and advice.
