Job Description
Home Instead Senior Care in Somerset New Jersey, is looking for a CLIENT CARE COORDINATOR to become a part of our team and join our mission of enhancing the lives of aging adults throughout our community. Home Instead Senior Care provides a variety of services that allow seniors to remain in their home and meet the challenges of aging with dignity, care and compassion. The CLIENT CARE COORDINATOR must have a minimum of 2 years experience in home care, health care or a senior related industry. He/she is expected to perform a variety of duties that relate to client care including answering telephone inquiries, care consultations with potential clients and family members, and quality assurance visits.

The CLIENT CARE COORDINATOR must have a minimum of 2 years experience in home care, health care or a senior related industry. He/she is expected to perform a variety of duties that relate to client care including answering telephone inquiries, care consultations with potential clients and family members, and quality assurance visits.
Responsibilities:
- Answer telephone inquiries from prospective clients / family members / referral sources
- Conduct Care Consultations with prospective clients / family members to discuss their needs and provide solutions
- Monitor and log both client and CAREGiverSM activity; including follow-up on CAREGiver assignments and client service
- Conduct client/CAREGiver introductions as needed
- Develop and maintain positive relationships with clients/family members through quality assurance visits and phone calls
- Work with the Director of Nursing and other team members to coordinate various aspects of client care
- Organize and participate in semi-annual CAREGiver evaluations
Qualifications:
- Two years experience in home care, health care or senior-related industry required
- Must have computer skills and be proficient in Word and Excel
- Must have the ability to work evenings or weekends as required
- Possess a valid drivers license and auto insurance
