Position Summary

Plans, coordinates and provides total patient care specific to diagnosis and age of patients served on a continuing basis, to obtain optimum health for patient and family, in accordance with accepted hospital standards and policy and the South Dakota Nurse Practice Act. Consistently employs patient and family-centered care to promote safety, quality and satisfaction.

• Contributes to the ongoing assessment of assigned patients utilizing age specific growth and development guidelines. Recognizes changes in the patient’s health status on a regular basis as evidenced by chart review/observation.
• Assists with patient/family teaching.
• Demonstrates the ability to appropriately assess patients, apply clinical interventions, evaluate outcomes, and implement change to the plan of care.
• Demonstrates the ability to recognize an emergency and initiate appropriate response.
• Initiates, confirms, or assists in the authorization process for all appointments, treatments and diagnostic imaging in conjunction with patient navigators.
• Performs nursing procedures and treatments according to nursing and unit specific standards, as evidenced by documentation and observation.
• Prioritizes incoming phone calls and faxes from patients/families and other medical personal and walk-ins.
• Screens patients/families for multidisciplinary service needs.

Education

Requirements: Active LPN license required.
