The caseload will not necessarily be Bullhead, Kingman, Fort Mohave proper, but dependent on the need case managers may be asked to travel anywhere throughout Mohave County. The majority of travel is to member’s homes, nursing homes or assisted living facilities, with occasional travel to doctor’s offices (to retrieve records) or quarterly council meetings or team meetings. There are case managers that to work in the field 2-3 days a week, doing as much documenting from the field and may spend the remainder of the week working from their home office, documenting and returning member calls.
 
This position requires a person to have a reliable car (Concur for mileage) and reliable internet service connection, physically able to carry/bring out equipment (lap top, portable printer), organizational skills, ability to work autonomously completing assessment visits by due dates outlined by state regulator

 
Primary Responsibilities:


Assessment of members to identify unmet needs, coordinate services, community resources, and treatment needs  

Delivering a holistic approach to coordinated care based on the enrollee needs

Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file

Creating, reviewing and revising Plan of Care plans and focusing on disease management

Functioning as an advocate for members and decreasing unnecessary hospitalizations

Collaborating with the member's PCP to deliver and coordinate necessary services

Building relationships with members and their families; assisting them with proper health choices

Ensuring cases are documented in a timely manner

Requirements

Required Qualifications:



Bachelor's level degree in Social Work or other related field

2+ years of experience in providing case/care management services to persons who are elderly and/or person with physical or developmental disabilities

1 year experience in long-term care, home health, hospice, public health, assisted living, and or working with the elderly and physically disabled

Basic level of proficiency in Microsoft Programs (Word, Excel, Internet, Outlook)           
Preferred Qualifications:


Medicare/Medicaid experience

A background working in geriatric special needs

A background in behavioral health

Experience with electronic charting
