RN Case Manager

A San Francisco Bay Area Medical Group is seeking a number of Outpatient Case Managers to work throughout the Bay Area in their outpatient multispecialty clinics.

The RN Case Manager will be part of a Medical Group which has worked in the region for more than 50 years, and is known for their quality of care and the quality of life provided to their employees.

In collaboration with the Director of Case Management and Manager of Care Coordination, the Nurse Case Manager will be responsible for collaborating with healthcare providers and physicians for problem solving, prioritizing and facilitating patient care objectives, the coordination and development of patient education, and discharge patient education and follow-up for the area assigned.

The Utilization Nurse will be an employee of the Medical Group, but will work in a series of outpatient facilities, providing case management and problem solving to physician groups. Ideal candidates will have 2 years experience in case management, and will be able to work independently, ideally with time management skills and a positive attitude.
