Home Care Registered Nurse, LTAC

Multiple positions now exist following recent staff advancements for a Home Care RN with a reputable and expanding hospital system in Western Massachusetts! This area of the state offers beautiful natural outdoor attractions and some highly desirable communities in which to live and work.

The Home Health Nurse will not-for-profit, 250+ bed hospital system that includes multiple locations and service lines throughout the community. Services include a 24-hour emergency room that treats nearly 35,000 annual patients, cancer center, bariatric facilities and pain management center. The medical system has received national distinction in multiple disciplines including cardiac care and trauma.

This LTAC Home Care Nurse will be responsible for assisting patients and their family members with care management in the home setting. The Home Health Care RN will provide explanation of treatment and care, administer medication, record symptoms and share critical information with patients and their families.

This Long Term Care RN will treat patients recovering from a variety of injuries and illnesses within the home including stroke related and post-surgical care cases. Operating with a large degree of autonomy, the Home Care Registered Nurse will report directly to the Nursing Manager and provide patient care within a designated territory.
