The Care Management Nurse Coordinator will be part of a medical center recognized as the premier health care provider in the surrounding community. In collaboration with department directors, the Care Management RN Supervisor will plan, organize, direct, and coordinate nursing service in designated patient care areas in accordance with applicable state and federal guidelines, regulations and standards. Directed by the Nursing Director of Case Management, the Supervisor of Care Coordination will assure the quality of patient care for specific patient groups.

The RN Supervisor of Utilization Review and Discharge Planning will be responsible and accountable for unit systems functions, staff performance, and patient care outcomes. The Care Coordination Nurse Coordinator will serve as a liaison among all disciplines including Administration, Medical staff, HIM, Nursing, and Finance.

The Care Management Nurse Coordinator will work collaboratively with the health care team to achieve best practices, discharge planning, clinical documentation and utilization management functions. The Supervisor of Case Management will follow regulatory licensing and accreditation standards applicable to the UR function. Demonstrates thorough working knowledge of applicable regulations and guidelines while ensuring that utilization review procedures comply with applicable regulatory and accreditation standard.
