Summary:

1. A highly functioning Team Care Assistant organizes the Provider/TCA interaction into four components allowing maximum efficiency of data collection, chronic disease surveillance, medical decision making, and patient education. The TCA performs a variety of clinical and clerical activities to expedite the patient flow and business activities of the medical office. The Certified Medical Assistant gives injections/immunizations on a routine, regular basis as patients' needs dictate.

Responsibilities:

Assists in the maintenance of a therapeutic environment that promotes effective and cost-efficient patient care and that provides for the safety of patients, staff, and visitors. 1. Possesses knowledge about safety policies of the office, as well as the established policies and procedures of the organization. 2. Works independently within the established guidelines of the practice. 3. Responsible for keeping exam rooms and lab stocked. 4. Actively participates in the cleaning and sterilization of instruments, if indicated. 5. Assists the office nurses in scheduling patients for tests or procedures. 6. Assists the nurse case managers in opportunities to promote healthy preventative services by tracking and collecting appropriate data 7. Assists in setting up refills on physician work list if indicated. 8. Must be comfortable documenting and verbally presenting all of the information collected from the patient to the provider in the exam room with the patient. 9. The TCA will notify the Provider of any alerts in the patients electronic record. 10. Team care assistants must act in a supporting role to collect data and enter documentation that accurately reflects the service on a specific date of service under the supervision of a licensed provider. 11. The TCA completes all documentation of the entire office visit, including the Physical Exam and requested testing/procedures on behalf of the Provider, and includes everything said during the course of the visit. Assists physician and nurse as needed with office procedures and exams. 1. Possesses knowledge and skills necessary to give IM injections/immunizations. 2. Possesses knowledge and skills necessary to assist physician and nurse in performing office procedures or examinations. 3. Possesses knowledge and skills necessary to perform various clinical tasks such as EKG's and venipunctures, depending on the practice. 4. Demonstrates knowledge and skills necessary to provide care appropriate to patients of all ages. 5. Participates in learning new procedures, policies, equipment and treatments. 6. Utilizes appropriate resources to instruct patients and family members of their diagnoses and medications. 7. The TCA will work as a positive team member with the Provider and other TCAs and attend and participate actively in Team meetings, which will review disease symptoms and terminology, learn and share how to deal with difficult patients, and process issues that need discussed to improve patient care and satisfaction. Assists the nurse and physician staff to assure quality health care and a smooth, orderly flow of patients through the office. 1. Assists nursing staff by preparing the patients for exams or office visits by taking vital signs (height, weight, pulse, and blood pressure. 2. Assists nursing staff by screening patients for general medical, and by obtaining lab specimens from patients, if needed. 3. Supports physician and nurses in their day to day duties by sharing necessary information to assure the delivery of quality health care to patients. 4. Possesses flexibility in the work environment in order to promote a smooth, orderly flow of patients through the office in a timely and efficient manner. 5. Takes and documents a competent, complete initial history (the HPI, and ROS) using Family Team Care tools, also includes updating the electronic record of current health maintenance procedures, Meaningful Use requirements, as well as the Quality Metrics. 6. All information regarding the patients current disease state, progress, and new symptoms are collected and recorded. 7. A good understanding of the review of medications, regimens, adverse reactions, and assessing compliance of meds is an essential key to obtaining a thorough history, in order to understand potential barriers to the patients care 8. Records in the appropriate fields of the electronic record and reports to the Provider all the labs, specialty visits, ER or hospitalizations, since the last visit 9. Assists and/or performs prescribed procedures in compliance with medical provider orders. 10. Conducts, performs, and labels appropriate laboratory tests 11. Completes job responsibilities in a professional and timely manner. 12. Demonstrates technical knowledge of equipment and its use, and has good typing and spelling skills. Assures that patient education is provided when appropriate, in a caring and professional manner. Assists the clerical staff in assuring optimal workflow and quality patient care. 1. Provides instructions to patients when necessary on preparing for tests, surgery, etc. 2. Assists the nursing staff with patient education involving a wide variety of health related subjects. 3. Demonstrates knowledge of the principles of growth and development in order to assess data reflective of the patient's status and interpret appropriate information required identifying each patient's age specific needs. 4. Participates in continuing education by attending seminars and various meetings. 5. Assists the clerical staff by answering the telephone and taking messages; scheduling appointments for surgery, tests and office visits; retrieving charts for the day; filing any papers or charts that need to be filed. 6. Assists with a full range of front office operations as required if required by office. 7. Assists the clerical staff by copying and collating medical records and patient education materials. 8. Competent in the use of all office computer systems 9. Provides detailed patient education and review of the office visit, including but not limited to: assessment findings, medications, tests and results, and future care plan of the patient. 10. The TCA will complete the office visit by giving and reviewing the clinical summary with the patient.

Education: 1. Trade School in the field(s) of medical assisting preferred Credentials: 1. CPR 2. Valid Drivers License in the state of IL 3. Other: Certified as CMA medical assistant. 4. Other: Team Care Certification/Training. Experience: 1. 1-3 years of experience in/with Clinical and clerical experience in a physician office, computer knowledge, detail oriented and have the ability to work with a wide variety of people preferred.
