Reporting to the Clinical Support Services Manager, the Palliative Care Specialist assists in in planning, developing, implementing, coordinating and evaluating palliative care services in a manner consistent with the Mission of St. Bernardine Medical Center. Included in this responsibility is the development and maintenance of strong relationships with the care team in delivery of palliative, comfort care to seriously ill and dying patients. This position is accountable for those functions, which enhance the delivery of palliative care through such activities as education; quality improvement activities; consultation and care management. This position is responsible for promoting high quality, cost effective care for palliative care patients served in a highly regulated and fiscally challenging managed care environment. In addition this position is responsible for developing and maintaining relationships in our community for both referral and resource management (Hospice, Home Health, Nursing Homes). The Palliative Care Specialist assists in providing support as needed to the Cancer Care Center; assists in providing direct patient coordination, guidance, education, referral and follow up for the Cancer Care Center for patients with breast conditions. This position requires the full understanding and active participation in fulfilling the Mission of St. Bernardine Medical Center. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support St. Bernardine Medical Center's strategic plan and the goals and direction of the quality improvement/process improvement activities.

Requirements: Minimum 3 years of clinical experience or a comparable combination of education and experience required Palliative care, Oncology, Hospice or Home Care experience preferred Graduate of an School of Nursing; Pain Resource Nurse Certification within two years of employment in the job. Knowledge of: principles of nursing process and team process; clinical standards for hospice and palliative care practice; death and dying process, terminal/chronic illness, grief, coping skills, pain and symptom management; community resources related to palliative care or hospice programs; Medicare and MediCal regulations, Title 22, NHPCO and The Joint Commission standards related to palliative care; governing bodies rules and regulations; reimbursement and financial management concepts; concepts of managed care; effective problem-solving techniques including conflict management skills; excellent oral and written communication skills; CQI and QA methodologies; computer skills Bilingual/Spanish preferred Current California RN license; BCLS
