Physical therapists at MedStar Health VNA make a positive difference in people's lives by creating innovative solutions to meet patients' individual rehabilitation needs. This patient-first approach leads to improved quality of life and independence at home. Home healthcare PTs have the autonomy to make individualized patient-centered decisions to promote optimal clinical care.

Job Details:

Provides assessments to determine patients' rehabilitation needs and psychosocial status.

Assesses each patient's environment and recommends adaptive equipment and/or home modifications as needed.

Establishes an effective physical therapy treatment plan in partnership with the patient, their family/caregiver, the patient's physician and other members of the health care team; modifies the treatment plan as needed.

Provides physical therapy services in accordance with the treatment plan, agency standards, and regulatory guidelines.

Helps patient achieve independence at home through activities of daily living training, functional activity training, visual/perceptual training, and psychosocial interventions, and heat/cold application.

Provides patient and/or caregiver education regarding the injury/disease process, functional limitations and the purpose of treatment interventions. Trains patients and/or caregivers in aspects of care that will be managed independently after discharge.

Plans adequately for discharge including referrals to appropriate community resources.

Communicates regularly with the patient's physician, home health care team, and other key members of the care team.

Qualifications

EDUCATION: Bachelor's degree in Physical Therapy from an accredited school.

EXPERIENCE: 1 year clinical experience.

LICENSE/CERT/REG: License to practice Physical Therapy in the District of Columbia, Maryland or Virginia. CPR certification required.

SKILLS: Good clinical skills. Knowledge of home equipment, home care reimbursement, and regulations governing PTAs. Verbal and written communication skills. Basic computer skills preferred.
