You will perform home visit treatments to patients for our client Home Health Agencies doing diagnostic testing and measurements, develop and implement care plans, continue to assess and revise said plans, educate and instruct clients and family members, document observations, interventions and patient evaluations. Be able to perform patient evaluations, re-assessments, re-certifications and OASIS Discharges.
