Sutter Physician Services, an affiliate of Sutter Health, is partnering with Sutter Care at Home to provide telephonic advice services to home health patients.

The RN Triage Nurse for Home Health is a part of the health care team who provides virtual care to patients via the telephone. The main accountabilities of this position include triaging calls to the most appropriate level of care, providing symptom management advice and self-care intervention, identifying resources, consulting and referring, taking after-hour calls and performing nurse follow-up activities.

The ideal candidate for this role is one who has a passion for palliative care, is empathetic, and compassionate. In addition to being a great listener, you must be a strong communicator, have the sense to know when to ask more or less questions and be respectful and professional in your clinical care telephone demeanor. You must demonstrate and exercise independent and sound judgment to assist patients with complex and diverse issues effectively and efficiently. Lastly, you will be required to utilize and navigate proficiently through an electronic health record; the ability to learn this technology will be required.

Minimum Requirements:

* Non-restricted board certified Registered Nurse (RN). In addition, you must obtain and maintain a non-restricted California RN license through the Board of Registered Nurses. Sutter Physician Services will pay for applicable fees for the initial licensing process.

* At least 1 year of clinical experience providing direct patient care.

* At least 1 year of clinical experience in one of the following areas: advanced chronic illness, palliative care, hospice care (other home care based experience may be considered in lieu of hospice).

Preferred experience/skills:

* Bachelor's Degree in Nursing.

* Knowledge of state and federal hospice and home health regulatory guidelines.
