Home Instead Senior Care-Evansville is looking for a qualified candidate to function as a Quality Assurance CAREManager.  Our company employs a large pool of CAREGivers that provide non-medical services for seniors in their homes.  The CAREManager performs a variety of duties in the area of client services (case management and quality assurance).

Responsibilities include:  
  • Conduct Quality Assurance home visits and phone visits
  • Act as primary contact person for client and family members regarding any service concerns/issues
  • Conduct Care Consultations and phone inquiries with prospective clients
  • Assess client needs and create Individualized Service Plan
  • Establish and nurture relationships with referral providers in the community conduct
  • Conduct Client-CG introductions, assist HR and Scheduling with client-CG compatible matching, provide HR feedback on CG performance, answer incoming calls
  • Participate in weeknight After Hours On Call rotation, act as emergency fill-in CG as needed 
 
Qualifications:
  • Strong interpersonal and communication skills
  • Assessment and treatment planning skills
  • College education preferred. Prefer individual with social work/human services background and training
  • Ability to function as a team player in a fast-paced office environment, while meeting individual accountability goals
 
Involves travel in Vanderburgh, Posey, Warrick & Gibson counties, with access to company vehicle for some of the travel. Non-exempt position with paid holidays, paid vacation, minimum essential medical and dental plans offered.  General working hours are Monday-Friday 8a-5p.  please submit online application with Resume and Cover Letter, atn: Christy Wallace, Operations Manager.
