Responsibilities:

* Has knowledge and understanding of daily assignments and unit activities.

* Answers call light in a timely manner to determine needs and accepts assignments from the rn.

* Performs routine tasks and simple treatments under the direction of a licensed nurse.

* Assists with the admission, transfer, and discharge process.

* Performs finger stick blood sugars and documents on flow sheet.

* Collects specimens, labels and takes to the lab.

* Conducts rehab groups as per the unit schedule.

* Continuously observes patients' behaviors and activities.

* Performs rounds on patients in accordance to unit policy.

* Escorts patients when leaving unit for activities and scheduled tests.

* Notifies rn of any changes or unusual occurrences in patient condition,(agitation, aggression, response to pain, changes in alertness, unusual skin redness or tears, etc).

* Performs behavior management procedures in relation to the age and size of patient under direction of the rn.

* Documents care and rehab group activities according to charting procedure.

* Provides personal hygiene and comfort measures to patients as directed and documents in medical record.

* Obtains vital signs, heights, weights as needed, provides complete or partial baths, including showers and oral hygiene.

* Documents hygiene and comfort measures in the medical record.

* Performs catheter/perineal care or changes as needed.

* Positions, ambulates and transfers patients.

* Assists patient in use of assistive devices (walker, wheelchair).

* Responds to patient needs promptly by making rounds frequently and anticipating patient needs.

* Assists in the maintenance of adequate nutrition, hydration and documents in the medical record. Serves between meal snack/supplements as per unit schedule or as ordered.

* Prepares and assists patients for meals, distributes trays and nourishments, water, assists with feeding patients as necessary.

* Documents type of diet, fluids (i&o's). Follows the patients diet order.

* Assists patients in participating in social dining program.

* Maintains a clean and safe environment for patient and self.

* Uses proper handwashing techniques and uses standard precautions at all times.

* Removes all unnecessary equipment from patient's room. Keeps bed in low position at all times with wheels locked.

* Demonstrates effective and accurate verbal and written communication.

* Reports off to charge nurse before leaving work area.

* Communication is timely, accurate, concise, complete and appropriate.

* Demonstrates age specific knowledge for the patient population of his/her unit.

* Greets family and patients in a pleasant and courteous manner.

Qualifications:

Because you're committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That's why you'll bring to your role the right set of qualifications:

Education: Must have a high school diploma or GED.

Certifications: Certified Nurses Aides must have current certification from state of Oklahoma. Certification in AHA BLS. Certification in hospital approved behavior management program.

Other: Must be familiar with procedures involved in patient care tasks.
