Expert knowledge of industry accepted criteria e.g. Star Hospital Measures, CMS and Health Plan guidelines and guideline application
Clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating)
Supervises a team of Community RN Care Managers providing transition of care coordination for Medicare members coming home from an acute care or sub-acute inpatient stay.
Provide support to staff with focus on three HEDIS measures:  Hospitalization for Potentially Preventable Complications (HPC), Plan All Cause Readmissions (PCR) and Medication Reconciliation Post Discharge (MRP).
 Extensive coordination and collaboration across regional M&R operations; national UCS and OPTUM Clinical Programs to provide a seamless service to members during their transition home and beyond.
Adjust workflow and responsibilities according to business and operational needs
Oversee day - to - day operations of the Community RN Care Managers
Conducts values and performance coaching which includes: annual reviews, corrective action plans, and recognition of employees
Efficiently navigates and utilizes computer data entry programs including but, not limited to: Microsoft Office (Outlook, Word, and Excel)

Requirements

Required Qualifications:

Active Unrestricted RN License for your state of residence
3+ years' of experience as Case Manager
1+ years' of clinical management experience of 10 or more
Efficiently navigate Microsoft Office (Word, Excel, Outlook)
Preferred Qualifications:

BSN or MSN
Complex Case Management experience
2+ years' of clinical management experience of 10 or more
CCM – Professional Certification or Licensure
