Senior Health Coach - Part Time 28 hours - Central, MO

The role of a JOIN for ME Health Coach is to facilitate and coordinate the successful launch, delivery and completion of a new Childhood Weight Management program in Missouri. The twelve month program delivers evidence-based content through local community partners and the JOIN for ME Health Coach, in a group setting for kids, teens and their parents. The Health Coach will drive program success benchmarks including participant engagement, enrollment, and attendance with the goal of achieving participation throughout the duration of the program. This person will actively create community awareness through partnerships with United Healthcare Community Plan, providers (Pediatricians, Primary Care Providers), schools (nurses, principals, superintendents) and other community partners. The Health Coach will also work in close partnership with Network Partners and UnitedHealth Group leadership to ensure that the program successes and opportunities are documented and shared.
 
Primary Responsibilities

Lead the promotion of the launch in Missouri to achieve enrollment targets by the enrollment deadlines as well as retention throughout the duration of the program
Collaborate with internal leadership teams to ensure optimal product utilization
Develop and maintain files of community resources for augmenting program as needed
Lead and Coach JOIN for ME classes in Central Missouri area and surrounding areas
Partner with local community resources to create awareness of and enrollment channels into the program
Partner with project team to ensure all necessary metrics are tracked and communications delivered throughout the course of the program
Work directly with the participants and their caregivers to respond to ongoing questions about program management and encourage participation
Aid in the training and ongoing support of other local coaches to ensure program development is communicated to coaches and the family friendly style of the program is well represented and modeled
Deliver Parent / Caregiver Information Sessions and Classes
Collect & filter participant success stories to share with the client
 

Required Qualifications:


Bachelor's degree
3+ years experience in wellness related field
Group Facilitation experience
Ability to travel within Central MO to deliver group health coaching classes
Experience with computer and web-based applications to manage consumer data (including Microsoft Outlook, Microsoft Office)
Ability to work non-traditional and flexible work hours as needed to promote and partner for success of the program launch
Preferred Qualifications:

Bilingual Spanish
Experience in Childhood Obesity

Bachelor's degree related to health and wellness

Certified Health Education Specialist

Certification in a nationally recognized coaching program
