Home Instead is currently seeking a talented, compassionate individual for an exciting opportunity providing non-medical, in-home care management. You will be making home visits on a regular basis to conduct ongoing care plan assessments, acting as an advocate for our clients and families, connecting them with resources and conducting training to our CAREGivers with the goal of providing a better quality of life for our clients through our service!
- Supervise and coordinate admissions, assessments, and care plans to assure accuracy and completeness.
- Communicate with clients, families, employees, staff members and other providers.
- Provide various types of training to our CAREGivers to help them develop critical skills.
- Work collaboratively with a team of professionals to provide quality care, support and education to clients and their families.
- Must be well organized, have good communication skills and have strong time management and multi-tasking abilities.
- Timely completion of required documentation.
- Strategic and analytic thinking to address gaps in care.
- Participate in care planning, discharge meetings and other meetings as requested.
Requirements:
We are seeking a Registered Nurse or LPN (must have at least one year of post graduate relevant case management experience) or Social Worker (must have a minimum of two years of relevant case management experience). Other related disciplines such as Nurse Practitioners, Professionals with a BA or MA in Gerontology, Professionals with a BA or MA in Human Services will be considered based on their level of relevant experience on a case by case basis.
Computer skills and resources:
- Ability to use a variety of electronic information applications/software programs, electronic medical records experience
- Intermediate to Advanced computer skills and proficiency with Microsoft Word, Outlook, and Excel, excellent keyboard and web navigation skills
