Providence is calling a full-time, evening shift Home Health Registered Nurse ( RN ) to Providence Saint Joseph Home Health in Burbank, CA.
Our goal is to give our patients the education and resources needed to regain and maintain good health as well as to help them remain in the comfort of home and in the community.

In this position you will:
* Apply the nursing process to develop and execute an age-appropriate plan of care for assigned home care/hospice patients by coordinating the interdisciplinary team to implement and/or update the patient plan of care according to assessment findings.
Required qualifications for this position include:
* Current unencumbered California Registered Nurse ( RN ) License
* Current Basic Life Support BLS certification for Healthcare Providers from the American Heart Association.
* Valid Driver's License
* Minimum 1-year of RN experience
Preferred qualifications for this position include:
* Recent Home Health experience is strongly preferred
* BSN
* EMR experience

From same-day referrals and a smooth transition from hospital to home, to attentive follow-up and communication with your own doctor, we provide care and support for you every step of the way to help restore your health to its fullest potential.
