Overview:

Under the direction of the Director/Manager or Clinical Coordinator, the registered nurse performs duties in accordance with the orders of a patient's physician, the policies and procedures of the Health System and Home Health and accepted standards of practice for their profession. Performs related duties as assigned.

Responsibilities:

Makes initial and on-going skilled nursing visits; provides nursing assessment, treatment and patient/family education as ordered by a physician in a timely manner. Responsible for development of the plan of care under the direction of the physician, updates care plan as needed.Completes all patient and non-patient related paperwork in required time frames.Communicate changes in patients; conditions and needs to physicians, other health care providers and staff as appropriate and participates in related patient conferences as needed.Responsible to ensure that physician's orders have been obtained prior to delivery of care.Counsels patients and their families in meeting the patient's nursing needs.Participates in in-service programs, seminars, and co-worker meetings to maintain and update nursing skills as appropriate.Provides Home Health Aide care supervision as appropriate and in accordance with statutory regulation.Reads and becomes familiar with appropriate policy and procedures; nursing practice exemplifies knowledge of this policy and procedure.Practices nursing in a manner that at all times protects the safety of patients and their families.Maintains productivity per Home Health standards.Demonstrates the ability to prioritize assignment based on patient need and satisfaction.Establishes a good rapport with patients and their families, Health System staff and other individuals in the community.Utilizes scripting as appropriate.Responsible for participating in customer service initiatives.Ensures care delivery and documentation are in compliance with current Federal and State regulations and JCAHO standards, as well as standards of nursing practice.Attends departmental meetings and participates in committee work as requested.Consistently adheres to universal precautions including use of personal protective equipment as appropriate.Maintains a professional appearance, adhering to Health System dress code, consistently displaying Health System name badge.Adheres to Health System and Home Care attendance standards.Performs other duties as assigned.

Position Qualifications:

* We require a nursing degree from an accredited school.

* You'll also need a current RN license in our state.

* Basic life support training should be completed in 90 days with advanced training required at certain intervals.

* Experience: Minimum of two years recent nursing experience.

* Other: Have comprehensive knowledge of general nursing theory and practice. Ability to work independently. Must be flexible, motivated and have excellent written and oral communication skills.

The employee must be able to lift and/or move more than fifty pounds. While performing the duties of this job, the employee occasionally works in outdoor weather condition. The noise level in the work environment is usually moderate. Employee may experience exposure to wild and/or domesticated animals. Physical demands are attached plus push, pull, and grip up to 50 lbs., pinch up to 10 lbs., kneel, crouch, and crawl minimally.Preferred Experience: Home care experience preferred.
