POSITION SUMMARY
The Field Case Management nurse acts as liaison between the claims adjuster, the injured worker, the physician, employer and other parties involved in the injured workers treatment plan. You will obtains treatment plan and work status information and provides updates to all parties with the goal to assist with returning the injured worker back to a productive lifestyle.
This is a dynamic, fast paced position that varies from day to day. It requires excellent time management skills and ability to multi-task, as well as some flexibility in scheduling as appointments may occasionally fall before or after typical business hours.
The Field Case Manager role will provide the ideal candidate with the ability to problem solve and work as a team on a daily basis to provide optimal outcomes for their clients. Schedule is flexible during the day depending on appointments.
It can provide a high bonus potential!
Fundamental Components:
* The Field Case Manager will travel 50 - 75% to attend appointments to meet with injured workers, physicians, employers, physical therapists and other parties involved in treatment plan.
* Approximately 25% of time will be spent on clerical activities to update all parties, make calls to coordinate treatment needs, and to create reports and letters, and to attend occasional marketing events to establish stronger relationship with accounts.
BACKGROUND/EXPERIENCE
* 2 to 5 years RN clinical practice experience.
* Ability to travel within a designated geographic area for in-person case management activities
* Bilingual preferred
* Knowledge of laws and regulations governing delivery of case management services is highly preferred
* Job-specific technical knowledge, (e.g., knowledge of workers compensation)
* Case management is a plus
* Any exposure to workers comp or home health experience is highly preferred!
* Nursing experience to include ER, CCU, ICU, Home Health is a plus!
* Strong computer skills, Windows, Word, Outlook, smart phone and strong typing skills needed!
EDUCATION
The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.
LICENSES AND CERTIFICATIONS
Nursing/Certified Case Manager (CCM) is desired
Nursing/CRRN - Certified Rehabilitation Registered is required.
