Job Description Details:

Performance Improvement

* Participates constructively in PI activities in the department

* Can discuss performance improvement activities for the department

* Contributes ideas for cost reductions, improved efficiency and outcomes without sacrificing quality.

* Maintains awareness of cost containment and waste reduction practices.

* Maintains and uses material, supplies and equipment in a cost-effective manner.

* Encourages and contributes innovative ideas and ways of doing things that increase efficiencies by challenging oneself and others to not accept the status quo.

* Recommends changes to policies, procedures and environment to enhance everyone's ability to provide optimum service to all patients, visitors and employees.

* Demonstrates understanding of PDSA (plan, do, study, act.) process and Just Culture.

* Keeps abreast of regulatory requirements for assigned unit.

Order Processing

* Processes Physicians orders into computer system.

* Accurately reviews, interprets and evaluates physician orders based on populations served.

* Follows appropriate policies and procedures for entering protocol orders (ie. pneumovax,

* influenza, etc.)

* Selects medications on Order Entry based on CMS guidelines when necessary (ie. epoetin, immune globulin)

* Monitors the patient profile for allergies, renal function, therapeutic duplications, interactions, incompatibilities and possible adverse events when verifying physician orders.

* Communicates effectively with clinicians on the patient's behalf when orders appear unclear or ambiguous.

* Documents all interventions per policy and procedure.

* Evidence of follow through is traceable. For example, if an intervention is documented, the patient's electronic chart contains evidence of said intervention to communicate to the other caregivers.

* Reviews, interprets and evaluates physician orders in order to prepare, calculate and dispense appropriate dosage forms based on populations served.

* Schedules medications according to the dietary precautions pertaining to the medication's administration (aware of policy and follow through).

* Checks order against generated label, profile or report to ensure accuracy (the 5 "rights" of medication dispensing/administration).

* Checks medications retrieved by technicians, prior to dispensing.

* Dispenses all non-ADM (Pyxis) medications with a label

* Identifies and documents on occurrence reports or via Meditech, all medication errors, "near misses" or adverse drug reactions for management review and follow-up

Equipment: Pyxis Medstations, PyxisTM CII Safe

* Uses equipment and software according to manufacturers' guidelines

* Willingness to learn/teach about new equipment and/or computer software.

* Assists nursing and pharmacy technical staff to ensure that all Pyxis equipment is in working order.

* Capable of the following:

* Loading a medication into a medstation (at the medstation and the console).

* Unloading a medication from the medstation.

* Knowledge of policies and procedures related to all Pyxis equipment.

* Troubleshooting a failed drawer or Medstation.

* For controlled substances, capable of:

* Receiving medication from wholesaler into CII Safe

* Sending to medstations

* Returning to the CII safe

* Expiring or wasting

* Utilizing inventory function

* Compounding a controlled IV infusion

* Participates in the Quality Assurance tasks surrounding the Pyxis Medstations on a

* regular basis.

* Checks medications for accuracy prior to and after packaging by a technician or another

* pharmacist

* Orders supplies as needed i.e. thermal printer and laser printer supplies, toner, paper and labels.

Clinical Skills/Activities

* Communicates effectively with physicians and nurses to clarify orders that are

* questionable.

* Able to obtain needed information for providing drug information.

* Documents appropriately when clarifications are needed and logs interventions as per pharmacy policy

* Capable of utilizing the appropriate technology for drug information or for dosing calculations

* Employs independent thinking for clinical issues

* Assists other pharmacists and interns with profile reviews and interventions.

* Consults the Clinical Coordinator for interventions as needed.

* Participates in clinical activities that support patient care and services

* Consistently participates in all clinical programs according to policies established by the department including but not limited to:

* Therapeutic Interchange

* IV to PO Program

* Renal dosing of all medications included in the department policy.

* Non-formulary medication policy

* Restricted drug policy

* Adverse drug reporting.

Parenteral Solutions / Calculations

* Ability to prepare parenteral solutions and perform calculations pertaining to those solutions.

* Practices aseptic technique in the preparation of IV's.

* Capable of accurately calculating solution drip rates, necessary dilutions and the resulting concentrations.

* Demonstrates the ability to work with and instruct IV personnel in the preparation of IV

admixtures.

* Knowledgeable about any special considerations for unique IV preparations, i.e. need for filters, proper hood for preparation, when not to shake preparation, etc.

* Demonstrates the ability to independently research the correct dilutions and compatibilities when preparing an IV for the first time.

* Participates in the calculation and/or preparation of Chemotherapeutic Medications, as necessary.

Time Management

* Capable of prioritizing and completing work/projects in an efficient manner

* Effectively manages time to complete assigned work during shift.

* Assists or offers to assist other staff members in completing work assignments, as needed and as time allows (ie. Pyxis refills, pre-packaging medications, crash carts, etc.).

* Provides timely and accurate dispensing of medications

* Able to prioritize responsibilities (ie. complete work on assigned floor including any and all problem orders needing to be resolved)

* Willing to participate in other departmental activities such as patient counseling,

* QA/Performance Improvement activities, etc

Other duties as needed and assigned by the manager..

POOL/PRN position. Rotating Hours scheduled by manager and determined by the needs of the department and hospital census.
