POSITION SUMMARY
The Field Case manager will make assessments and attend physician, therapy appointments, meet with employers and injured workers, amd coordinate treatment as approved. You will work to coordinate return to work when released by physician for the members.
This role in FL requires a CCM, COHN, CRRN, or CDMS.
This position is 75% in the field and the other time at home, and needs to be in the Port St Lucie area up to Melbourne area. This role does offer some flexibility in your schedule, and will be full time hours Monday-Friday.
You will work from home but daily travel in and around the area. Schedule is flexible during the day depending on appointments. High bonus potential.
Fundamental Components:
* You will attend appointments and follow the case of the injured worker.
* You will need to use an I-phone, and computer for email and documentation
* You will need strong organizational skills.
BACKGROUND/EXPERIENCE
* Must have current FL RN in good standing.
* Must have strong clinical background with minimum of 2 years of RN experience.
* Must have good analytical skills and critical thinking.
* Must have adult nursing assessment skills.
* Home Health, prior Workers Comp/Case Management/Rehab experience a plus.
* Bilingual preferred but not required.
* CCM, COHN, CRRN, OR CDMS is required. One of these is a requirement.
* Strong computer skills including MS Office are needed!
EDUCATION
The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.
