Job DescriptionThe Registered Nurse (RN) will:
- Coordinate and monitor medical, dental, nursing and clinic services
- Monitor individual/patients for signs and symptoms of acute illness, complications and exacerbations of chronic illness; refer for medical care as indicated individual/patients on caseload.
- Provide ongoing review of all lab/test results, and medical appointments
- Ensure follow up on all prescribed and recommended treatments
- Participate, as needed, in clinical/staff meetings to provide medical and nursing information
- Provide on-site nursing visits for ongoing individual/patient health assessments and review of applicable medical records
- Handle other duties, as needed
- 1+ year of Nursing experience
- Associate's Degree in Nursing
- NYS Registered Nurse (RN) license
- Solid assessment, clinical, and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
- Able to multitask efficiently and effectively
- Bachelor's Degree in Nursing
- Previous experience working with the IDD population
