General Summary of Duties:  Responsible for developing and implementing all clinical operational functions.  Responsible for overall management of the Agency in the absence of the President/CEO.
 
Supervision Received:  Responsible to the President/Chief Executive Officer.
 
Supervision Exercised:  Directs the efforts of the Managers of Clinical Service, Manager of Paraprofessional Services, Manager of Therapy and Medical Social Work Programs.
 
Typical Physical Demands:Requires prolonged sitting, requires some standing, bending or stooping.  Requires eye-hand coordination and manual dexterity.  Requires the ability to lift a minimum of 20 pounds.  Requires the ability to ascend/descend stairs.  Requires the ability to distinguish letters, numbers and symbols.  Requires corrected vision and hearing to normal range.  Requires use of telephone, office equipment and copier.
 
Typical Work Conditions: Work is performed in an office environment.  Contact with administrative and direct service staff.
 
Essential Duties:  (This list may not include all of the duties assigned.)
 
1.   Ensures implementation of all clinical services in accordance with Agency policy.
a.   Ensures that all departments are integrated and present a seamless delivery system to Agency customers.
b.   Develops the clinical management team; acts as coach and mentor to management staff in clinical department.
c.   Ensures that qualified personnel are employed to carry out work of Agency.
 
2.   Works collaboratively with the Vice President of Quality and Performance Improvement to ensure that all clinical services are delivered within parameters established by state and federal regulations and accreditation standards.
 
3.   Identifies opportunities for Agency to support the community health care system goals.
a.   Develops programs consistent with Strategic Plans, to meet community needs.
b.   Facilitates change and restructuring in clinical area to meet customer demands.
c.   Ensures optimal staffing is in place at all times.
d.   Monitors productivity, clinical metrics and holds Managers accountable to achieve established goals.
 
4.   Supports the Agency's commitment to performance improvement.
 
5.   Collaborates with the Senior team to develop Agency bridge goals and initiatives.
 
6.   Participates in the development of the Strategic Plan and quarterly evaluation process.
 
7.   Establishes and maintains relationship with community health organizations to ensure Agency is part of the delivery system and remains abreast of all marketing opportunities.
 
8.   Ensures own professional development through participation in educational seminars.
 
9.   Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
 
Ancillary Duties:
 
1.   Performs other duties as necessary to support the Mission, Purpose and Strategic Plan of the Agency as directed by the President/CEO.
2.   Must have access to reliable transportation to travel between multiple work sites; and travel is required in order to perform essential duties.
 
HIPAA and Privacy:  Properly direct any complaints and/or grievances as indicated per VNHSC policy.  Comply with HIPAA regulations, specifically the Privacy and Security Rules.  Protect personal health information (PHI), electronic personal health information (EPHI) and financial information by limiting the use and disclosure of health information and financial information to the minimum amount necessary with only those who must know how to get the job done.  Follow the department’s specific policies and procedures to protect health information.  Help one another to ensure confidentiality at all times. 
 
Knowledge, Skills and Requirements:  Knowledge of home health and hospice care delivery system, with in-depth knowledge of reimbursement.  Knowledge of state, federal and accreditation  regulations governing the delivery of home health and hospice care.  Proven success in fast paced environment; ability to facilitate change and integrate systems within diversified Agency.  Excellent oral and written communication skills.  Possesses adequate transportation to carry out job requirements.  Must show proof of satisfactory automobile insurance with minimum coverage to be determined by agency policy when using auto for transportation.
 
Education:Bachelor of Science in Nursing and Master's Degree in Health Care Administration or related field required.
