Health Coordinator - Full-time / Part-time

-responsible for contacting members to inform and educate them on medical management programs
-conducting research, performing data entry, responding to inquires from members
-supporting the clinicians in the Medical Management department with their provider and member activities
- developing written correspondence to members and to other department personnel and coaching skills to educate members on medical issues.
-The position is phone based and the employee is responsible for receiving incoming calls
-Duties will include: conducting Health Risk Screenings, Special Beginnings Assessments, providing community resources, and assisting members with general concerns. Customer service is responsible for reviewing member's benefit information.
