Position Summary

Implement medication use activity and clinical services designed by the Director or designee and implement cost containment measures involving drug utilization in the hospital. Ensure accurate and appropriate dispensing of medications, accurate documentation, provides accurate and complete drug information and provide staff and physician education as appropriate.

Minimum Education Requirements

  • B.S. or Pharm.D. degree

Minimum Experience Requirements

  • One-year experience as a pharmacist preferred.

Licenses/Certifications

  • Licensed in Colorado

Position Duties (Essential Functions, including % of time)

Medication Assessment

  • Interprets, evaluates, and monitors patient medication orders in a timely manner with a conscientious degree of accuracy and care using the patient medical record and / or pharmacy profile.
  • Evaluates all orders for appropriateness and safety.
  • Works with physicians and departments to ensure quality
  • Participates in emergency alerts to ensure adequate drug supply and appropriate drug use.
  • Demonstrates high technical skill level in utilizing a variety of pharmaceutical competencies including, but not limited to, calculations, aseptic technique, cytotoxic drug properties, vancomycin / aminoglycosides, infectious disease, TPN, and Pharmacokinetics management.
  • When appropriate, works with hospice and other outpatient services to ensure adequate drug supply and appropriate drug use.
  • Monitors the effects of medication therapy in hospital patients and shall be an active participant in patient medication counseling.
  • Participate in medication reconciliation per facility standard

Medication Distribution

  • The pharmacist will coordinate acquisition, distribution, and storage and security of pharmaceutical products and coordinates the ordering, manufacture, and distribution of medications in the hospital.

Collaboration

  • Provides consultative, clinical, and educational service the health care professionals to provide high quality, cost-effective use of pharmaceuticals.
  • Provides drug information to the health care team by verbal communications, in-services, memos, and newsletters to ensure appropriate prescribing and administration of medications.
  • Provides drug information to patients and families, by verbal and written communication to ensure understanding of prescribed medications.
  • Provides age-appropriate patient counseling to foster patient understanding and optimal outcome as required.

Problem

  • Recognizes and resolves inappropriate orders for medications using knowledge and effective communication with the health care team.
  • Prioritizes daily workload demands by balancing dispensing requirements with time requirements for the resolution of questionable orders and unit rounds.
  • Attains professional growth by attending or participating in continuing education programs annually.
  • Demonstrates ability to remain calm and provide appropriate care in emergency situations.
  • Participates in interdisciplinary patient care rounds.

Leadership

  • Participates in Medication Use Evaluations as established by the Clinical Coordinator.
  • Participates as a member in interdisciplinary teams maintaining communication and information lines between the pharmacy and other health care professionals.
  • Actively participates in quality improvement efforts focused on optimizing the department's overall operation and performance. Professional role model for all staff, demonstrating high technical ethical / legal professional pharmacy practices.
  • Delegates to non-pharmacy personnel in accordance with state Board of Pharmacy and hospital policy.
  • Provides orientation to new employees.
  • Actively supports management in establishing standards of practice, noting non-compliance, and participating in corrective action plans.

Documentation

  • Maintains patient and other records using pharmacy computer and manual systems to ensure timely and accurate patient billings and compliance with regulatory agencies.
  • Maintains documentation of clinical activities and facilitates maintenance of adequate supplies.
  • Documents clinical interventions and interactions with health care providers in clinical documentation program.

Physical Requirements

  • Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
