What You Will Do:
As a Registered Nurse and On Call Coordinator with Amedisys, you will coordinate and evaluate patient care during designated on-call hours. Your responsibilities would include answering incoming telephone calls from patients/patients' families and referral sources; making patient care decisions in response to telephone calls; coordinating and assigning visits to home care/Hospice providers which identifies as medically necessary to meet patients' needs; and accepting reports from and responding to questions from home care/Hospice providers who are making visits.
- Respond incoming telephone calls and other inquiries from patient/patients' families, referral sources and other healthcare professionals.
- Document incoming patient telephone calls, response or direction, and the coordination of services using the appropriate processes and systems
- Make patient care decisions in response to incoming patient telephone calls, using current patient data bases and professional nursing judgment. Such responses may include giving advice or direction to the patient/cg contacting the physician or scheduling a visit.
- Provide guidance and direction to home care/hospice providers regarding patient care and clinical issues, symptom management, patient incidents and/or injuries.
- Regularly communicates with and works in cooperation with appropriate personnel to coordinate patient care services.
- Oversees weekend, after hours and holiday schedules, as prepared by branch office and makes adjustments to schedule in response to patient and and/or staffing changes.
- May perform the duties and responsibilities of the Intake Coordinator position and may assign admission visit to appropriate staff when weekend/holiday admission is indicated.
- Obtains authorization for Admission or PRN visits added to the weekend or holiday schedule, as required by the payer.
Required:
- Current and unencumbered license to practice as a Registered Nurse in all states in the assigned territory.
- Three (3) years of experience as a registered nurse while one of those years must consist of full-time experience in providing direct patient care in the home health/hospice setting.
- Current CPR certification
- Able to assess patient status and identify requirements relative to age specific needs.
- Valid Driver's License and reliable transportation and agency required liability insurance to complete home visits.
Preferred:
- Bachelor of Science in Nursing (BSN) degree
- Two (2) plus years home health/hospice experience
- Acute care experience as a registered nurse with the adult population in a medical – surgical environment
