Nurse Navigator - Orthopedics Patient Management

Dignity Health / Northridge Hospital Medical Center

The primary responsibility of the Orthopedic Nurse Navigator is to demonstrate leadership in delivering and improving care delivery through collaborative strategies. The Orthopedic Nurse Navigator is responsible for the coordination and management of care for the Orthopedic Service Line. The incumbent is also responsible for facilitating and assisting patients through the healthcare continuum, enhanced communication among caregivers and lateral integration of care. Takes a lead role in coordinating with a multidisciplinary approach to planning and providing care, acting as a liaison between the surgeon, PCP, and other ancillary practitioners and departments including Radiology, Laboratory, Pain Management, PT, OT and Surgery to ensure that the needs and concerns of the orthopedic patients are addressed. The Navigator should possess a specialized knowledge in influential leadership. The Nurse Navigator is accountable in meeting clinical, financial and performance improvement outcomes for the orthopedic patient population. Under the general direction of the Service Line Director of Orthopedics, the Orthopedic Nurse Navigator plans, directs, and evaluates patient healthcare and outcomes in both the Inpatient and Outpatient care delivery settings.

Qualifications

Minimum Requirements:

* Current CA RN license required.

* CA NP license strongly preferred.

* Minimum five years nursing experience, including care of the orthopedic patient.

* Two years experience in education or program development position strongly preferred.

* Graduate of an accredited School of Nursing, with a minimum of a BSN required. MSN with advanced practice certification strongly preferred.

* Must have working knowledge of performance improvement, licensure and accreditation requirements.

* Demonstrate effectiveness in teaching, organizing, planning and developing programs.

* Strong interpersonal communication and customer relation skills required, with excellent written and verbal communication skills.

* The ability to be self-motivated and use effective problem-solving techniques required.

* Experience working in a Center of Excellence setting strongly preferred. Strong knowledge of Microsoft Office (power point, excel, word, etc.), Outlook, and Internet.

* Certified Orthopedic Nurse required or ability to obtain certification within one year of hire/transfer.

* AHA BLS required.
