Job Summary

Provide skilled, comprehensive care to the terminally ill patient/family/caregivers as prescribed by the physician and in compliance with the State Board of Nursing regulations, the patient, and organizations policies and procedures. Oversee and provide primary care, including all clinical care, palliative care and symptom control. Identify other physical, psychological, social and spiritual needs for assigned patients. Develop and implement the Plan of Care to meet the patient/family/caregiver needs, initiate intervention and support, and re-assess/ re-evaluate the patients needs ongoing, to provide comprehensive and appropriate care. Work in collaboration with the Clinical Services Manager and Interdisciplinary Group (IDG) in the planning, implementation and evaluation of medical and clinical care. Act as a role model providing leadership and support to hospice staff. Manage related expenditures in a fiscally responsible manner in accordance with Banner Hospices budget.

Essential Functions

* Collaborate with IDG members to provide continuous assessment of assigned patients, initiate appropriate nursing actions, and evaluate the results of these actions.

* Facilitate the implementation of nursing interventions, preventive and therapeutic nursing procedures. Provide direct patient care as necessary.

* Assume a leadership role to facilitate interdisciplinary coordination and collaboration.

* Provide appropriate information and explanation to patients family/caregivers. Facilitate patient and family education.

* Evaluate the effectiveness of teaching to promote quality patient/family/caregiver outcomes, and ongoing professional development of hospice staff.

* Develop visit frequencies in accordance with patients needs and acuity, reviewing at least weekly and adjusting as needed.

* Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.

* Facilitate the quality assessment, performance improvement (QAPI) process.

Minimum Qualifications

Must possess knowledge of case management or utilization review as normally obtained through the completion of a bachelor's degree in case management or health care.

Requires current Registered Nurse (R.N.) license in state worked. For assignments in an acute care setting, Basic Life Support (BLS) certification is also required.

Requires a proficiency level typically achieved with 5 years clinical experience with at least two years current hospice, home health, medical/surgical, or critical care experience. Must possess knowledge and understanding of hospice philosophy, principles of death and dying. Must have a working knowledge of care management, acute care and/or home care environments, community resources and resource/utilization management. Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills. Must demonstrate ability to work effectively in an interdisciplinary team format.

Bachelors degree and CHPN (Certified Hospice and Palliative Nurse) preferred. Experience with pain and symptom management interventions preferred.

Preferred Qualifications

Additional related education and/or experience preferred.
